School Health Requirements

PreK3 & PK4 yr programs: Physical
Kindergarten: Physical, eye exam by optometrist or ophthalmologist, dental exam.
2nd Grade: Dental Exam
6th Grade: Physical and dental exam, *Tdap-1 immunization, and *Meningococcal-1 (*new 2020-2021)

IDPH new vaccine requirements for 2021-2022 school year: requirement consists of one dose of meningococcal conjugate vaccine (MCV4) for 6th grade entrance, and two doses of the vaccine for 12th grade entrance. If the first dose of MCV4 vaccine was given on or after the 16th birthday, then only one dose is required. This is a progressive requirement.

All students in grades 5,7, 8 that will be participating in sports need to have a "sports physical on file in the office.  If the student is a 6th grader they will NOT need a separate sports physical.  A"sports physical" does not fulfill the school physical requirement.  A school physcial must include proof of immunizations.
